Confidentiality: Protecting your Privacy
Protecting your privacy is important to us. We treat all of your personal and health information with strict confidentiality.
How much information do I have to give you?
- We need your name and date of birth.
- We would like to have a photo of you for identification purposes, but you can take the
picture yourself on your phone. - If you share your cell phone number with us, we can text you important documents. (if
you don’t have your own phone, we can complete the paperwork here at the center) - We do not need insurance information.
- It is helpful but not mandatory to show us your photo ID.
- Our services are at no-cost to you, so we do not need payment card details.
Will people be able to call and find out that I was there?
- No. We will not confirm or deny your presence at our center.
Do you have a privacy policy?
- Yes. You will be sent a copy of our privacy policy as soon as you make your first
appointment. - Intake documents, including the privacy policy, will come to you as a text message. Read
and sign the documents before you arrive if possible.
May I bring a support person?
- Yes. We encourage you to bring your partner, a family member or a friend.
- When you arrive, we will meet with you alone to verify your safety and desire to have
the person present for discussions about your personal health details. - You get to decide how much, if any, your support person can participate.
- At any time, you may request to meet privately with our staff.
Will I have to take paperwork home with me?
- No. In order to protect your privacy and confidentiality even after you leave, all our
documentation can be paperless.
Your information is safe and secure with us and we won’t share it without your written permission.
