Is my visit confidential?

Protecting your privacy very is important to us. All of your personal and health information stays confidential.

How much confidential information do I have to give you?

We will ask for:

  • Your name and date of birth.
  • Your phone number to confirm your appointment.
  • One form of identification.
  • We do not need payment or insurance details.
  • Our services are performed at no-cost to you.

Can someone call and find out I was there?

  • No. We will not confirm or deny your presence at our center. Your information remains secure and confidential.

Do you have a privacy policy?

  • Yes. Our privacy policy is available to you online or at the Center.

May I bring a support person?

  • Yes. We encourage you to bring your partner, a family member or a friend.
  • During pregnancy tests appointments, guests are not permitted. For ultrasound appointments, one approved guest is allowed for moral support.
  • We will meet with you individually first to ensure your information remains confidential. With your approval and at the proper time, your guest will be invited back.
  • At any time, you may request to meet privately with our staff.

Will I have to take paperwork home with me?

  • No. It is up to you if you choose to take things with you.
  • In order to protect your privacy, all discharge documents can be paperless.

Your confidential information is safe and secure with us.

We will not share without your written permission.

  • At your appointment, you may complete a Release of Information to authorize the release of your health information to your medical team.
  • You can request necessary medical information anytime after your appointment. Ask your medical team to fax CPC an authorization.
